ACUTE CARE PROGRAM

What is?

Following a hospitalization for injury or illness, many patients require continued medical care, either at home or in a specialized facility. Post-acute care refers to a range of medical care services that support the individual’s continued recovery from illness or management of a chronic illness or disability.

Post-acute care includes rehabilitation or palliative services that beneficiaries receive after or in some cases instead of, a stay in an acute care hospital. Depending on the intensity of care the patient requires, treatment may include a stay in nursing homes, also called skilled nursing facilities, which provide a wide range of health and personal care services.

Florida nursing homes are serving an increasingly diverse patient base and providing a greater variety of acute care, rehabilitation and convalescent services that cannot be delivered elsewhere.

About as...

Our project starts with the main purpose of  providing an extra level of assistance and continuity of care. Our team eases the transition between healthcare providers by better aligning our physicians, hospitals and post-acute care settings according to the patient’s needs. This collaboration is designed to streamline the transfer of care and reduce hospital readmissions.

Important that you know…

Our primary purpose is to bridge the gap between hospitalization and returning home or to a lower level of care. It aims to provide comprehensive and specialized services to promote recovery, manage medical conditions, and enhance overall well-being. The goals of post-acute care include:

01

Continuity of Care:

Post-acute care ensures a smooth transition from acute care settings to a post-hospitalization environment. It ensures that patients receive the necessary medical attention, rehabilitation, and support to optimize their recovery.

02

Patient and Family Education:

The post-acute care team educates patients and their families about the individual's condition, treatment plan, and self-care strategies. This empowers patients to actively participate in their recovery and promotes long-term health management.

Who Benefits from Post-Acute Care?

Post-acute care is also beneficial for individuals with pulmonary and other chronic medical needs. Our team has the resources and expertise to provide comprehensive medical care and monitoring tailored to the specific needs of each patient.

By accessing post-acute care, individuals with complex medical needs can receive ongoing medical management, medication administration, and diagnostic services. This level of specialized care helps to optimize their health outcomes, prevent complications, and enhance their overall quality of life.
